Accepting these terms
These Terms of Use ("Terms") govern your use of livingwaterspharmacy.com, portal.livingwaterspharmacy.com and any related online services (together, the "Services") operated by Living Waters Pharmacy ("Living Waters," "we," "us"). By using the Services you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Services.
No medical advice
Information on this website, including descriptions of medications, articles and answers to frequently asked questions, is for general education only. It is not a substitute for advice, diagnosis or treatment from a licensed healthcare provider. Always talk with your prescriber about whether a medication is right for you, and never delay or ignore professional medical advice because of something you read here.
Using the Services does not by itself create a pharmacist–patient or physician–patient relationship. Our pharmacists remain available to answer questions about medications we dispense to you.
Prescriptions and eligibility
- All of our medications require a valid prescription from a licensed prescriber who has a legitimate relationship with the patient.
- Our screening tools and intake results are not a diagnosis and do not guarantee that a prescription will be issued. The decision to prescribe belongs entirely to the prescriber.
- Our pharmacists may decline to fill a prescription when, in their professional judgment, doing so would not be safe or lawful.
- We dispense only to patients located in states where Living Waters Pharmacy is licensed.
- You must provide information that is true, complete and current, including your full medical history and current medications. Inaccurate information can put your health at risk.
About compounded medications
Living Waters is a 503A compounding pharmacy. Compounded medications are prepared for an individual patient based on a prescription. Compounded medications are not FDA-approved, and they are not generic equivalents of brand-name drugs. The FDA does not verify the safety, effectiveness or quality of compounded medications before they are dispensed. We prepare our medications under applicable state and federal pharmacy standards, including USP requirements for sterile compounding.
Results vary from person to person. We do not promise any particular outcome, such as a specific amount of weight loss.
Online intake form
When you complete our intake form, you confirm that you are the patient (or are legally authorized to act for the patient), that the information you provide is accurate, and that you agree to the consents shown in the form. Intake information is reviewed by a licensed prescriber and our pharmacy team and is handled as described in our Privacy Policy.
Provider portal accounts
The provider portal is for licensed prescribers, their authorized staff and Living Waters personnel. If you have a portal account, you agree to:
- keep your sign-in details and verification codes private and not share your account;
- use the portal only for patients you are authorized to treat, and only for legitimate medical purposes;
- submit prescriptions only within the scope of your license and in compliance with applicable law;
- keep your professional details (license, NPI, clinic information) accurate; and
- tell us promptly if you suspect unauthorized access to your account.
We may suspend or close an account that is misused, inactive or no longer authorized. Portal activity is logged for security and compliance.
Orders, shipping and returns
Prices, availability and shipping times can change and are confirmed when your order is processed. Temperature-sensitive medications are shipped in protective packaging; please store them as directed as soon as they arrive.
For patient safety, medications cannot be returned or exchanged once they have been dispensed, except where required by law. If there is a problem with your order, such as damage in transit or a dispensing error, contact us right away and we will work with you to resolve it.
